    Description/Qualifications

    Our broad-range, full-scope Palliative Care program recently celebrated its 8th year in service. The highly collaborative group operates as a cohesive interdisciplinary team that is focused on providing supportive care to patients and families referred to Palliative Care.

    • Interdisciplinary team includes: Physicians, Nurse Practitioners, Social Workers, Nurse, CNA, and CMA, as well as Chaplains and Ethicist.
    • The Palliative Care Team works closely and collaboratively with all other disciplines in the inpatient setting including: attending providers, specialty providers, support staff, nurses, therapists, and care managers.

    We are seeking a MSW Care Manager to facilitate new patient encounters, patient follow-ups, advanced care planning, goals of care conversations, and other interventions related to the promotion of quality of life.

    • Serves as a member of the interdisciplinary Palliative Care Team.
    • Inpatient clinician, averaging 5-10 patient encounters/day.

    Through communication and available resources, the MSW promotes appropriate utilization, high quality care and cost effective outcomes that align with the key strategies of the Care Team.

     

    You may be an ideal fit if you:

    • Hold a Masters of Social Work from a school accredited by the Council on Social Work Education.
    • Are able to apply to be registered with the State of Oregon within 30 days of hire as either: Licensed Master’s Social Worker; or, Clinical Social Work Associate; or, Licensed Clinical Social Worker; and can obtain registration within one (1) year of hire.
    • Have training and/or experience in Palliative Care.
    • Are a highly collaborative team player, and can maintain performance in a fast-paced environment.
