    Description/Qualifications

    SHACC is an outpatient clinic that provides monitoring, medication adjustment, and comprehensive education for patients on anticoagulation medications.  As a nurse-run clinic, SHACC collaborates with outside providers within the state of Oregon to safely manage patients on warfarin and low molecular weight heparin.  Using an in-depth protocol, SHACC navigates patients through beginning anticoagulation, peri-procedural management, and routine supervision of their blood levels and medication doses. 

     

    What you'll need:

    • BSN or non-nursing Bachelor's Degree with MSN. 
    • Minimum of two (2) or more years of acute care nursing experience required. 
    • Current unencumbered licensure to practice nursing as a Registered Nurse in the State of Oregon.
    • BLS from American Heart Association required.
    • Demonstrated good communication skills and the ability to teach anticoagulation procedures to patients.
