    Salem Health is currently accepting applications for an On Call Pharmacist.  


    The Pharmacist On Call implements and provides ambulatory pharmacy, inpatient pharmacy or discharge services as a member of the transition of care, Meds-to-Beds and/or clinic teams under the direction of Director of Pharmacy and/or in coordination with Salem Health Medical Group Providers. This position is responsible for managing the pharmacotherapy of complex patients.

    • Functional pharmacotherapeutic knowledge related to disease states managed.
    • Strong interpersonal communication skills, both written and verbal.
    • Leadership and cohesive team building skills.
    • Plan and coordinate own activities and those of others.
    • Collaborative approach to patient care.
    • Clear understanding of quality improvement process principles.
    • Ability to understand then translate or explain complex oral and written directions.
    • Analyze available information or data and formulate appropriate plans.
    • Present plans, presentations, etc. using Microsoft Office modules.
    • Familiarity with Epic or an alternative EHR



    • Graduate of ACPE accredited college of pharmacy required (Pharm.D. degree preferred, B.S. degree acceptable with applicable work experience or PGY-1 residency program completion).
    • Current Oregon Pharmacist license or eligibility to apply for Oregon Pharmacist license.
    • BLS from the AHA required within 90 days of hire.
    • Appropriate Board Certification (BCPS, BCPAP, etc.) is required within two (2) years of accepting position.
