    As an RN Discharge Caller I with Salem Health, you are responsible to initiate telephone calls to patients and/or caregivers to review discharge instructions, assist with care continuation, and improve patient experience and safety. The position requires a high degree of customer service and confidentiality, as well as a working knowledge of the programs and policies to facilitate exceptional customer experiences for every patient, every time.


    Core Functions

    • Review patient chart prior to each call: Eligible for call, working telephone number, MD Discharge Summary, discharge instructions/medications, pertinent clinical information related to most recent inpatient stay and conduct discharge follow-up calls
    • Contact the patient’s medical provider(s)/community resources if indicated to insure patient safety and remove barriers to care
    • Report pertinent data to other health team members and implements appropriate action if indicated
    • Assist RNs with daily problem solving of complex patient care issues that may include, contacting internal & external partners, locating patient resources and education, and escalating patient care concerns to improve care and remove barriers.
    • Conduct on-the-spot coaching and makes recommendations to improve quality of calls as needed.
    • Provide guidance and direction as needed to ensure RNs are actively engaged in Discharge Calling work with minimal distraction.
    • Ask for help and effectively communicates issues/concerns regarding work duties to the Manager without delay


    • Requires BSN or non-nursing Bachelor’s Degree with MSN for all external applicants for Salem Hospital, preferred for West Valley Hospital. Bachelor of Arts in Nursing also accepted.  For internal applicants preference may be given to applicant with BSN.
    • Minimum of one (1) year of acute care RN Staff Nurse experience required.
    • Minimum of five (5) years of nursing experience in an acute care hospital setting preferred.
    • Current licensure to practice nursing as a Registered Nurse in the State of Oregon.
    • Demonstrates ability to intervene appropriately in emergency situations
    • Demonstrates ability to identify and respond to patient safety concerns over the phone
    • Demonstrates the ability to ‘think outside the box’ in resolving patient issues
    • Demonstrates ability and willingness to manage conflict and mediate disputes when advocating for patients in support of the patient advocate role
    • Demonstrates knowledge of commonly prescribed & OTC medications, including effects, side effects and allergies
    • Demonstrates competence in navigating an electronic record to answer patient questions
