    Description/Qualifications

    The MSW Care Manager works with patients, families, physicians and community providers to evaluate resource/referral needs and coordinate discharge planning for our patients. They complete a comprehensive Psychosocial Assessment for each assigned patient, administers MMSE’s per doctor’s orders, and completes an evaluation for involuntary patients. MSW Care Managers provide supportive counseling to patients and their families in consultation with the treatment team.

     

    What you’ll need

     

    • Hold a Master’s Degree in Social Work from a CSWE accredited school.
    • Two (2) years of post graduate practice in a health care setting preferred. 
    • All MSWs must apply to be registered with the State of Oregon within one (1) year of hire into a position as either a:
      • Licensed Master's Social Worker or Clinical Social Work Associate.
    • Required to complete Crisis Prevention Intervention training within the first thirty (30) days after hire and annual recertification thereafter.
